if the fuel pressure regulator has completely failed, you will get fuel pouring from the vacuum lines. Could be the cold start valve leaking as well, Can you post a picture ?

same thing happened to mine. local garage replaced the fuel pressure reg. no problem since. cost $90, said it's a common issue.

Had this happen on an old e30, but it still applies. The diaphram in the FPR apparently ruptured and the vacuum line was sucking fuel into the intake instead of just vacuum. The engine was running terribly though due to extra fuel.

Can this symptom occur sporadically? OR, once it goes, the engine never runs right until replaced?

Once in a blue moon, my car will BARELY start, and then it runs rough for 10 to 15 minutes. This has not happened since June or July and has not happened since. At the moment, when I start it for its weekly idling session, it runs perfect. Thoughts?

Was it (the engine) warm when the hard start occurred? If so it could be your check valve in your fuel pump. My 735i had a hard start/rough running problem when the engine was warm and that was the cause.

If the car is stored, do you drive it? I wouldn't recommend idling it at all if you've layed it up for the winter. that's a recipe for more problems later.
