IIRC, automatic/steptronic is an option which comes with the premium package or bought seperately on its own.
The steering wheel looks very shiny in that picture, maybe the owner pot shoe polish on it or something.

Automatic/Steptronic is NOT part of the premium package. It is a stand-alone option...at least up to the 2004 model year. Had they made it part of the premium package in 2005?

I stand corrected. It is a seperate option for 1200 something bucks.

A very few X5 3.0's were made for the U.S. market with 5-speeds. Also unusual is the lack of automatic climate control (note the manual controls in the picture...most U.S. market X5's have automatic climate controls...

The 3.0 is the only one with an option for manual. I believe in MY 2004 they 3.0 started coming with a 6spd standard.
